STATEMENT of LIABILITY

With my signature, I hereby assume responsibilty for any personal injury to myself and others in my party, or for any loss or damage to my vehicle(s), equipment and personal property whether by accident, vandalism, acts of God or any other cause which may occur during my participation in this event, and hereby indemnify, release and hold harmless from any all claims made against Greater Meridian Chamber of Commerce and its officers, Raymond Trujillo Memorial Car Show, City of Meridian including employees at the event, as well volunteers and sponsors of the event on the date the event outlined at top of this form.

Additionally, with my signature I also acknowledge that I may receive text updates regarding this event and understand that my contact info will not be shared with any outside entities and will not be added to any group lists outside of this event.
